Consultant – Radiation Oncologist
Liver cancer treatment depends on tumour size, stage, liver function, and overall health. This comprehensive guide covers all treatment options available at Onco-Life: surgery, radiation, chemotherapy, targeted therapy, and immunotherapy. Learn what to expect at each stage.
Liver cancer (hepatocellular carcinoma or HCC) is one of the leading causes of cancer death worldwide. The encouraging news: multiple effective treatment options are available, and outcomes have improved dramatically with newer therapies.
Treatment success depends on early detection and access to advanced care. This guide explains every treatment modality available.
Liver Cancer Treatment Options Overview
Your oncologist will recommend treatment based on:
- Tumour size and number
- Cancer stage (local vs. advanced)
- Liver function (cirrhosis, fibrosis)
- Overall health and performance status
- Blood vessel involvement
- Multiple treatments may be combined for best results.

1. Surgical Treatment for Liver Cancer
Surgery offers the best chance of cure for early-stage liver cancer when a sufficient portion of healthy liver remains.
Surgical Resection
Removal of the tumour and a margin of healthy liver tissue. Success depends on the surgeon’s ability to remove all cancer while preserving enough liver for normal function.
- Best for: Early-stage tumours in patients with good liver function
- 5-year survival: 40-70% for early-stage disease
Liver Transplantation
Removal of the entire diseased liver and replacement with a donor liver. This treats both the cancer and the cirrhosis.
- Best for: Early-stage cirrhotic livers (within Milan criteria)
- 5-year survival: 60-80% – best long-term outcomes
- Challenge: Organ availability and lifelong immunosuppression
2. Ablation Therapy
Ablation destroys tumour cells using extreme heat or cold, without removing the entire liver.
Radiofrequency Ablation (RFA)
- Uses electrical energy to generate heat
- Best for small tumours (<3 cm)
- Minimally invasive – done under local anaesthesia
- Quick recovery – same-day or overnight stay
Microwave Ablation (MWA)
- Uses microwave energy – faster than RFA
- Less heat dissipation – more precise
Cryotherapy (Cryoablation)
- Uses extreme cold to freeze and destroy tumour
- Often combined with other therapies
3. TACE (Transarterial Chemoembolization)
TACE delivers chemotherapy directly to the tumour blood supply while blocking blood flow, starving the cancer of nutrients.
- Minimally invasive – done via angiography
- High drug concentration at tumour site
- Best for intermediate-stage disease
- May require multiple sessions
4. Radiation Therapy
High-dose radiation precisely targets the liver tumour while sparing healthy tissue. Modern techniques like SBRT (stereotactic body radiation therapy) allow higher doses with fewer side effects.
- SBRT (Stereotactic Body Radiation): High-precision, often fewer sessions
- Proton Therapy: Delivers radiation precisely to tumour, sparing liver
- Best for: Unresectable tumours, bridging to transplant
5. Systemic Therapies: Chemotherapy, Targeted, & Immunotherapy
Chemotherapy
Traditional chemotherapy drugs circulate throughout the body, killing rapidly dividing cancer cells.
Targeted Therapy
- Sorafenib (Nexavar): Blocks blood vessel growth
- Lenvatinib (Lenvima): First-line for advanced HCC
- Regorafenib: For sorafenib-resistant disease
Immunotherapy
- Anti-PD-1 antibodies: Atezolizumab, nivolumab, pembrolizumab
- Combination: Anti-PD-L1 + anti-VEGF (bevacizumab) – superior outcomes
- Mobilises immune system to attack cancer
- Best for: Advanced, metastatic disease
Immunotherapy has significantly improved outcomes in advanced liver cancer. Response rates of 20-40% are now achievable.
How Treatment Is Selected: The Multidisciplinary Approach
At Onco-Life, treatment decisions are made by a multidisciplinary team (surgeons, oncologists, radiologists, hepatologists) who review each patient’s imaging and lab work to determine the optimal approach.
The decision depends on:
- Barcelona Clinic Liver Cancer (BCLC) staging
- Child-Pugh score (liver function)
- ECOG performance status
- Patient preference and goals

Frequently Asked Questions
Liver Cancer Treatment Options: Complete Guide from Surgery to Systemic Therapies
Is Liver Cancer Curable?
Early-stage liver cancer is potentially curable with surgery or transplantation, with 5-year survival rates of 40-80%. Advanced disease is treated to extend survival and improve quality of life.
Which Treatment Is Best?
No single 'best' treatment exists. The optimal approach depends on tumour size, number, stage, liver function, and individual factors. Combination therapies often yield best results.
What Are The Side Effects?
Side effects vary by treatment. Surgery: pain, infection. TACE: fever, nausea. Systemic therapy: fatigue, diarrhoea, skin reactions. Your team manages side effects throughout treatment.
How Long Does Treatment Take?
Treatment timelines vary: surgery is a single procedure; ablation may take 1-2 sessions; TACE requires 2-3 sessions; systemic therapy continues until progression or intolerable side effects.
Can I Get A Second Opinion?
Absolutely. We encourage second opinions. Liver cancer treatment is complex, and multiple expert perspectives help ensure the best decision.
